The change plan worksheet is a way of writing down a plan so we have it in black and white. The worksheet encourages us to identify the changes we want to make and the reasons we want to make them. It asks us to list the steps needed, people who might be able to help and how, how we will know when the plan is working and any barriers that may get in the way of us following the plan.
When to Use This Tool
Having a plan is a very effective way to increase confidence. The change plan worksheet is a good tool for all participants to use but is particularly useful for participants who score high on the importance scale, but low on the confidence scale.
