How can I receive proof of my attendance at a SMART meeting?

You can use a printable form from our resources page for in person meetings, or the Pathcheck online tool for online meetings.

You can receive verification for any meeting on the listing page that has the orange tick, or blue Pathcheck tick.

For an in-person meeting:

To receive a signature you will need to participate in the meeting, not merely attend. This will be left to the discretion of the facilitator. Please follow the steps below to get verified.

1.Print off the attendance sheet and bring to the SMART Recovery meeting (Facilitators don’t have this form or printing facilities).

2.At the end of the meeting, the facilitator will sign the form. You will receive a signature for each time you attend.

For an online meeting:

Pathcheck is the online tool participants use to get a certificate of attendance after a SMART meeting. It’s especially helpful for those who need proof for court, parole, job agencies or support programs.

You can create a free Pathcheck account.

After attending a meeting, go to the Pathcheck website and enter the 6-character code. (The meeting facilitator should have shared this code in the Zoom chat.)

Use the same name and email when signing up and attending — that’s how the system matches you correctly.

Pathcheck certificates are only available for one week, so it’s best to access Pathcheck soon after the meeting. If you need verification of attendance for an online meeting after one week has passed, please email us at smartrecovery@srau.org.au. Please allow up to three business days for this request to be completed.

Your attendance will be verified automatically if you attended at least 75% of the meeting.

You can download your attendance certificate — or even email it directly to a case manager or court officer.

If you are still stuck after following these steps, the best option is to contact Pathcheck’s customer support directly — they’re usually quick to respond and can resolve most issues fast.
